We are located at 33 Madison Professional Park in Rexburg, Idaho. Go west up Main Street, past the last traffic light, and on up towards the Hospital. Just before entering the hospital parking lot, turn right onto Ash street and follow it south past where the road curves left and keep going straight up the hill into Madison Professional Park, 1 block to the side of Madison Memorial Hospital. We are at the top and on the left side of Madison Professional Park below Madison Park Dental in the lower level of the building. Follow a wide sidewalk next to our sign in the parking lot down to the door.
