About Our Practice
INJURY ASSESSMENT of the spine is complex and cannot be rushed. It is uncommon in a traumatic episode when a conscious individual is lying on the ground to have a significant spinal injury without severe pain, spasm, or tenderness. It is VERY COMMON , however, for an individual with a major injury, such as a herniated disc to have only mild or MODERATE pain and tenderness at the ONSET. The severity of pain, presence or absence of neurological symptoms, neck spasms, and tenderness can indicate when stabilization is needed Mobilization of the individual should be in the hands of trained personnel. Later, after resolution of the many ACUTE symptoms of inflammation and muscle spasm, when the patient walks into the office complaining of neck or back pain, it is relatively safe to assume that a SERIOUS spinal injury is NOT present. But because of the high incidence of exacerbation and referred pain, a quick scanning exam can be performed during observation to determine if other body parts need to be assessed. A history of the injury should include information on the mechanism of injury; extent of the damage (pain or disability) due to the injury, previous injuries to the area, and family history, if it has some bearing on this particular condition. In a spinal injury, the location of the pain (localized or radiating) , type of pain (dull, aching, sharp, burning), presence of sensory changes (numbness, tingling, or absence of sensation), and possible muscle weaknesses or paralysis are asked, noting how long it took for the patient to respond to the questions, in cases of concussion and memory loss. The observation begins immediately when the patient enters the room. Body language can signal such factors as pain, disability, and muscle weakness. The patients willingness or ability to move, general posture, ease of motion, and general attitude. Clothing and equipment may be removed to visualize any skin lesions. Posture is assessed sitting and standing, noting the head and neck posture. A scan exam assesses general motor function and rules out injury at other joints that may be overlooked due to intense pain or discomfort at the primary injury site. Active movement of the spine may be included prior to palpation. Gross motor movements for the spine include flexion, extension, right and left rotation, and right and left lateral bending. Palpation of the bony and soft tissue of the back and neck detect temperature, swelling, point tenderness, deformity, crepitus, muscle spasm, and skin sensation. Palpations are done sitting, standing and recumbent positions.
